Do you still say Asher Yatzar?

I have a question for fellow Jews with incontinence (if there are any others on here). We are taught to say the prayer Asher Yatzar after using and leaving the washroom, and as a part of morning prayers. If you are incontinent, do you still recite the prayer after, say, changing your diaper?

Due to COVID I haven’t seen my rabbi in a while and haven’t been able to ask him.

If you’re completely incontinence. When would you say it? Each time you wet?

There are alot of halachas involved in wearing diapers. I’m wondering if there is a shiur that I can listen to it answering all these questions

This is actually something I had thought about a few times on concept, despite myself not regularly saying the blessing even were I to use the washroom instead of my undergarments.

This being said, I did find a book [Encyclopedia of Jewish Medical Ethics: A Compilation of Jewish Medical Law on All Topics of Medical Interest by Fred Rosner] that seems the reference the question being asked here. However, finding a an electronic-publishing or pdf of the book seems a bit difficult, the closest being on Archive.org. If you ever find a reliable copy, or get an answer from your Rabbi, I would love to know - I’m certainly too shy to approach the topic with my Rabbi personally. ^^;;
